Both student and parent and/or guardian are required to sign this agreement prior to the start of any training in the Basic Samurai Cop self-defense Course.

As a student of the Samurai Cop self-defense Courses, I (__________________________________) understand that I am only to use what I learn in these courses for the purposes of Self-Defense to protect or defend either myself against a stranger attack or against anyone that may try and physically hurt me, or any member of my family. I also understand that any misuse of what I am shown in any of these courses will be grounds for immediate and permanent removal from the course and that I will not be allowed to return to the Shuyokan Dojo.

I also understand that participating in this course is a privilege and not a right, therefore I agree to always maintain good grades in school and complete all of my homework assignments.

I will always respect my parents, my family members, my teachers and my friends. I will not lie or cheat in school, and I will always keep my promises that I make to others.

As a Member of the Samurai Cop self-defense Courses, I understand that taking or using any type of illegal drugs, smoking any kind of cigarettes and/or consuming any type of alcohol is grounds for immediate and permanent removal from the course and that I will not be allowed to return to the Shuyokan Dojo.

As a Member of the Samurai Cop self-defense Courses, I also understand that belonging to any school and/or street gang is grounds for immediate and permanent removal from the course.

And finally, as a member of the Samurai Cop self-defense Courses, I will always conduct myself in a polite and courteous manner by always paying attention to the teacher, showing respect for other students, not disrupting the class in any manner, and by following all of the rules and regulations of the Shuyokan Dojo while attending any of the courses.

By signing our names below, I hereby acknowledge that both myself and my parent(s) or guardian have fully read, understand and agree to abide by the contents so stated in the Samurai Cop self-defense Student Code of Conduct Agreement.
